Swasthya Sathi Card – Form A, Form B, Form C, Form D
There are mainly four (4) types of Forms available on the Swasthya Sathi website and they are named as Form A, Form B, Form C & Form D. Each type of form has different purposes. We have discussed the purpose of each form and when it is required to be filled up by an applicant. So, read the article carefully –
Swasthya Sathi Form A:
Form A is required by someone who needs to include his / her family member names in the Swasthya Sathi Card. The form is to be filled up by individuals and following information will be required –
- Full Address,
- Name of the Beneficiary,
- Father’s Name,
- URN Number,
- Member Name,
- Sex,
- Age,
- Relation,
- Mobile Number,
- Khadhyasathi ID Number
- Aadhar Number (if any),
- Beneficiary Signature.
Swasthya Sathi Form B:
This form needs to be filled up during enrolment for Swasthya Sathi Scheme. If you want to avail the benefits of the Swasthya Sathi Prokolpo, then you must fill up this Form B completely to enrol your name and get your Card. Information that will be required to write in the Form B are –
- Applicant Name & Address,
- All Family Member’s Name,
- Mobile umber,
- Aadhar Number etc.
Include all the family members name in the “Members Area” of the form. After successful submission of the form and processing by the Dept of Health, you will be given a URN number and finally you will receive Swasthya Sathi Card.
Swasthya Sathi Form C:
Form C is required for Name Correction of members in Swasthya Sathi Card. If there is any mistake in your card, then you must submit the form duly filled to your nearby Duare Sarkar Camp. Following information will be required during filling up of the Form C –
- URN number,
- Existing Name of the family member,
- Details of the person or members whose name to be corrected,
- Name & Address of the applicant etc.
Swasthya Sathi Form D:
If you want to delete any of your family member’s name from the Swasthya Sathi Card, then you must submit the Form D. The details which are to be mentioned in the form D are –
- Existing Name, Age, Sex, Mobile Number, Aadhar no etc of the Members to be deleted,
- URN Number,
- Name and Address of the Beneficiary,
- Signature of the beneficiary.

To download the Swasthya Sathi Card from official website www.swasthyasathi.gov.in, follow the steps –
Step 1: Go to the official website of WB Swasthya Sathi Prokolpo which is www.swasthyasathi.gov.in.
Step 2: Now, open the “Apply Online” Link.
Step 3: After that, choose your desired form from the menu.
Step 4: Click over any form and a PDF version of the form will be opened.
