How to apply for Birth Certificate in West Bengal

Step 1: To apply for the birth certificate in West Bengal, one must go to the official website www.crs.wbhealth.gov.in.
Step 2: After that, click on Citizen Services > Birth > Apply for New Registration.
Step 3: Now, enter mobile number and click on “Get OTP“.
Step 4: Enter the OTP received on the given number and click on “Submit OTP“.
Step 5: Thereafter, birth registration form will appear and you will have to furnish the following information:
- Baby’s date of birth, gender and name,
- Place of birth – State, District, Block or Municipality, Panchayat and hospital type (Governemnt or Private),
- Name, email id, mobile number of the father,
- Religion and fathers education, father’s occupation,
- mother’s education details, mother’s occupation, age of mother during marriage
- Type of attention at delivery, Delivery method (Normal, forceps / vacuum or caesarian)
- Total babies count of mother, weight of the baby, pregnancy duration of mother.
Step 6: Upload all the necessary documents such as Aadhar Card, EPIC or Khadya Sathi and Discharge certificate and finally click on “Submit” button. After that, acknowledge number will be generated. Keep the acknowledgement no with you.

Birth Certificate Application Form Track
After successful application on the WB Helath CRS portal for online Birth Registration, the citizens will be provided with an Acknowledgement No.
The acknowledgement number will have to be entered in the Birth Certificate tracking link on the WB Health CRS website.
For this, go to the crs.wbhealth.gov.in and click on Citizen Services > Birth > Track Application.
Then enter your Application’s Acknowledgement Number and click on submit button.
